From the south:  Travel on US101, taking the first Garberville exit onto Redwood Drive.  Turn right onto Conger Street.  Our facility is located at the end of Conger Street.

From the north:  Travel on US101, taking the first Garberville exit. Turn left onto Redwood Drive.  Turn left onto Conger Street.  We are located at the end of Conger Street.

Jerold Phelps Community Hospital and Southern Humboldt Community Clinic are located at the corner of Cedar and Elm Streets, in Garberville, California. 

Hospital emergency entrance: 733 Cedar Street

Hospital visitor entrance: driveway is accessed from Elm Street 

Clinic, administration and accounting entrance: 509 Elm Street
